The One Health Quality CTO is a Care Transformation Organization (CTO) of primary care providers and practices with the shared goal of improving outcomes and lowering healthcare costs. Formed in 2019, our goal is to build team-based capabilities in primary care practices that will allow them to:
- Enhance health outcomes
- Improve the patient experience
- Reduce the total cost of care
