Behavioral Health Support
To benefit your patients with behavioral health needs:
- Make referrals to Adventist HealthCare’s behavioral health liaison, who can help schedule appointments and guide patients to support groups and workshops
- Join regular Physician Alliance teleconferences to discuss patient cases with psychiatrists and therapists
- Let us help your practice implement the screening, brief intervention and referral to treatment (SBIRT) approach for patients at risk of substance use disorders
Diabetes Management
The Adventist HealthCare Physician Alliance offers diabetes management support for patients who are newly diagnosed, have poor A1C control, or need additional education about their condition. The following resources are available to eligible patients:
- Diabetes Nutrition Class: In this free 90 minute class, concepts for better diabetes management through a healthier diet are introduced. Course content includes learning to identify foods that affect blood sugar and how well-balanced meals and snacks are key to optimal blood sugar control. Eligible participants must be under the care of a Physician Alliance provider.
- Tailored 1:1 Consultations: Physician Alliance patients attributed to a value-based contract are also eligible to receive individualized consultations and follow up with a certified diabetes care and education specialist.
- Gestational Diabetes Class: This class covers important topics to prepare expectant mothers to manage their blood sugar through monitoring, diet, physical activity and medication use. Click here for class dates and to register.

Weight Management
Give your patient access to comprehensive weight-loss support through CoreLife at Adventist HealthCare. This personalized program provides dietitians, exercise specialists and behavioral health counselors to help patients take charge of their weight. To make a referral, call 443-679-4309.
