Nurse Care Navigators
Improve the health outcomes of your most at-risk patients through care navigation services. When you participate in Adventist HealthCare Physician Alliance, you will have access to our valuable Care Navigation program team led by experienced registered nurses. The nurse care navigators will work collaboratively as an extension of your practice to reduce the burden on your staff to help improve the health outcomes of your most at-risk patients.
Role of Nurse Care Navigators
Nurse care navigators partner with your practice to drive down the cost of preventable hospitalizations and visits to the emergency department. These experienced registered nurses work one-on-one with your high-risk patients and their families, teaching them how to manage health conditions, get care and find local resources. Our team of nurses support your treatment plans by working with your patients to help establish better self- management of both acute and chronic conditions. Patients also learn how to access support from the Physician Alliance’s community partners, including social service agencies and nonprofit organizations.
Nurse care navigators with the clinically integrated network offers comprehensive care management services to high-risk and high-cost patients to help them achieve better health. The services include:
- Provide support through transitions of care
- Provide Disease management support
- Medication Management
- Medication reconciliation
- Provide Patient education on Health and wellness
- Address SDOH and barriers to care
- Coordinating referrals and appointments
- Assist with navigation of the healthcare system
You Stay Informed
To enhance patient outcomes, nurse care navigators meet regularly with your practice to review topics such as:
- Patient updates, hospital discharges and transitions of care
- Maryland Primary Care Program (MDPCP) and CRISP Reporting Services health data
- Quality metrics
- Opportunities to close gaps in care
- New resources
